Ask the Library: Reference

Most survey instruments are under copyright so you'll most likely need permission to use it, look for the following:

• Is there a copyright notice somewhere on the document or is it under a Creative Commons License?
• Are there instructions somewhere? Do they say anything about who can use the survey and for what purposes?
• Is there any contact information on the survey?

It's always good to ask first.
